Support program for employee health
promotion
LiSM 10!: Follow-up Program to
Prevent Lifestyle-related Diseases |
Though each company
provides periodic medical checkups
for their own employees, proper follow-up
and guidance based on the individual
data has not always been provided.
Effective support for worker's healthcare
will result not only in preventing
lifestyle-related diseases but
in reducing the costs of medical
insurance.
In collaboration
with the Physical Fitness Research
Institute, Meiji Yasuda Life Foundation
of Health and Welfare, we have developed
the LiSM 10! (Life
Style
Modification
by taking 10 minutes of counseling
on physical exercise and dietary habit)
program. This program focuses on health
promotion in physical activity and
nutrition after the medical check-ups
at each work site. The health promotion
program was develop on the scientific
basis of health promotion, behavior
science and health education. The
LiSM 10! consists of individual objective
setting to prevent lifestyle-related
diseases and individual counseling
by professionals once a month for
6 months to support the promotion,
recording of individual performance,
and support programs from worksites
and families of individuals.

An
intervention program was conducted
in 5 plants of two companies for adult
workers of 40's years from November
2001. The intervention group (83 workers)
used the newly developed program (LiSM
10!) while the control group (71 workers)
followed a conventional program in
which occupational health nurse provides
health promotion guidance after the
periodical check-ups. The performance
was evaluated in terms of dietary
behavior, physical exercise behavior
and risk-factors of lifestyle-related
diseases at 6 months after the intervention
initiated. Moreover, to observe if
the improved dietary and exercise
behavior could be sustained and if
the improvements could benefit against
the risk-factors, an additional survey
was conducted at one year after the
intervention program was completed.
Results of
6-month Intervention:
It was confirmed
that the LiSM 10! program significantly
and positively changed the behaviors
in physical exercise and dietary intake.
In addition, it was observed that
risk-parameters involved in lifestyle-related
diseases such as body weight, blood
pressure, cholesterol were significantly
improved. Hence, the LiSM 10! developed
was proved to be more effective and
useful than the conventional programs.
Results
of 12-month after completion of the
intervention program: It
was observed that positive changes
of the behaviors in physical exercise
and nutrition dietary intake were
maintained and that body weight,
blood pressure, blood glucose and
LDL cholesterol were improved
and maintained, but some of parameters
such as energy consumption, total
cholesterol and triglyceride were
rebounded. In summary, to maintain
the improved parameters after the
intervention, it is important to
provide a periodical follow-up counseling.
Expected Economical Benefit
in Reduction of Medical Cost:
A simulation study was conducted to
estimate the economical benefit of
this program. The study shows that
when the LiSM 10! is employed for
high risk group of 1,000 people for
5 years, 180 million JPY (1.7 million
USD) could be saved in medical cost
as compared with the conventional
programs.
On the basis of the series of intervention
study, the Manual of LiSM 10! for
Adults were developed. Another
intervention program using this manual
was started for white-collared
workers from November 2004, to
assure the effectiveness of the program.
We expect that the LiSM 10! Program
will be employed by health care unions
of private enterprises and medical
check-up organizations to prevent
lifestyle-related diseases of adults. |

10! for Elderly: Physical Activity
and Nutrition Program for the Elderly |

Aiming
to support "healthier longevity"
among elderly people and to reduce
the costs of the National Health Care
System, ILSI CHP Japan has developed
"TAKE 10! (Take 10 minutes for
physical exercise and Take 10 varieties
of foods) for the Elderly". It
is scientifically recognized that
people of all ages need appropriate
physical activity and must have
proper dieting habits. Different
from other programs, TAKE 10! for
the Elderly combined the two keys
in an effective way to sustain the
"self-help" lives of
the elderly.
A handout booklet and VTR material
have already been developed under
the supervision of TMIG (Tokyo Metropolitan
Institute of Gerontology) and experts,
and a manual for instructors will
be completed in February 2005. A unique
package consisting of those materials
and professional training in sites
will be introduced from April 2005
to promote self-help lives of the
elderly under local authorities.
An intervention
study was conducted for 1,400 elderly
people in the village of Nangai,
Akita prefecture, from July 2002
for one year. The nutritional intake,
physical conditions and psychological
status were evaluated through hematological
analysis, physical check-ups and
medical questionnaire.

Results:
The intervention study proved
that the TAKE 10! for the Elderly
could efficiently be introduced into
local communities and could effectively
maintain health conditions of elderly
people. The intervention program encouraged
the elderly to improve their dietary
habits, taking balanced diet including
more animal protein, and doing regular
physical exercise as the TAKE 10!
Booklet instructed. The improvement
resulted in increase of HDL cholesterol
and reduction in parameters of
arteriosclerosis. The improved
and balanced diet and regular physical
exercise maintained muscle strength
that was usually reduced along with
aging. This was shown in the fact
that the walking speed of the people
was maintained in the intervention
program against the aging. Moreover,
periodical participation in the program
encouraged the people to continue
physical activities and sustain the
behavior of physical exercise.
The TAKE 10! Program for the Elderly
is an effective tool to sustain the
self-help lives of the elderly.

| Low
Cost Application: The physical
exercise of TAKE 10! for the Elderly,
effective combination of stretch and
muscle training, can be started and
can be done on one's own at home or
at local community sites without expenditure
for training equipment. ILSI CHP Japan
is prepared to send trainers in collaboration
with TMIG to local communities to
train local trainers (community dietitians
and exercise advisors) who will provide
training and counseling to the elderly.
Thus TAKE 10! for the Elderly will
provide low cost application.
Reduction of "Bedridden"
Insurance and Medicare Expenditures:
Ministry of health, Labor and Welfare
in Japan released an estimate that
medicare expenditures for the elderly
would increase to the triple of
the current by 2025. We expect that
the TAKE 10! Program for the Elderly
would help in promoting healthier
aging by improving nutrition intake
and regular physical exercise for
elderly people, resulting in reduction
of "bedridden" insurance
and medicare expenditures.
Three national newspapers reported
the successful results of the intervention
study for the TAKE 10! for the
Elderly Program during October and
December 2004. More than 6,000 of
inquiries about purchase intention
for the program have been given
to ILSI CHP Japan and about 12,000
copies are sold. This indicates
strong interest in this program
among elderly people. We intend
to promote the TAKE 10! for the
Elderly in a systematic way on a
communities basis.
